Service Learning Forms and Info
According to Chicago Public Schools (CPS),
Service Learning is a teaching and learning methodology that connects classroom
curriculum with identified community issues and needs. The program engages
students in projects that serve the community and build their social and academic
capacities.
 
Graduation requirements:
Students must complete a minimum of 40 hours of service between 9th and 12th
grade. Sophomores must complete a minimum of 20 hours of service in order to
be promoted to junior status. Students must spend time preparing for and reflecting on the Service Learning experience.
In order to complete the graduation requirements for Service Learning hours, PMA students must adhere to the following: juniors from the class of 2013 and seniors from the class of 2012 can continue to earn individual Service Learning hours. They must document these hours using:
  • the pre-service preparation form
  • the post-service form
  • the MLA style, 1-2 page post-service reflection essay
  • the time sheet(s).

Note, the forms, sheets, and essay must be properly completed or no hours will be awarded.
 
Current changes in the CPS Service Learning requirement for graduation will directly impact freshman and sophomores from this school year, 2011-2012, forward. All Service Learning will be curriculum- and portfolio-based in addition to completion of the pre-service preparation form, post-service reflection form, 1-2 page MLA style essay, and time sheet(s) for student to be granted Service Learning hours towards graduation.
 
At PMA, students will be given the opportunity to complete their curriculum-based projects through either their JROTC, AVID, or Spanish class. They will receive either a regular project assignment grade or an extra credit grade.
 
Moreover, the portfolio includes a 3-5 page MLA style paper regarding the type of service completed and making sure to respond to the following: what is the type of Service Learning, who is impacted by this service, when does this service take place, who does this service, why is this type of service important and how is this type of service relevant to what was taught in the classroom. Types of Service Learning include: child and youth development, community development, culture, education, environment, food, hunger, nutrition, housing and homelessness, immigration, public health, public safety, and senior citizen support.
 
The portfolio must also include at least three of the following documentation: interview(s), pictures, brochure, PowerPoint, and published literature by the service agency where the Service Learning hours were completed.
 
Portfolios must be turned into the particular JROTC, AVID, or Spanish teacher responsible for the Service Learning projects for an academic grade. The teacher will then forward the portfolio to the Service Learning Coach. The Service Learning will then input the number of Service Learning hours specified by the teacher, no more than 16 hours for each portfolio project, to the student's Service Learning hours electronic record at the end of semester that the project was completed by the student.
