Service Learning Forms and Info
Welcome to Phoenix Service Learning Documentation Portal!
If you use this site effectively you will be able to plan, perform, and document you service learning project and guarantee that your hours will be tracked.
Each year of high School requires 10 service learning hours. The forms and procedures must be followed carefully or no hours will be awarded.
There are four easy steps to getting service learning hours.

Download all PDF versions of all forms listed below as work copies.

Complete Pre-service planning form and get approval from Mr. Saenz.

Take time sheet to all service learning activities and obtain signatures.

Complete Pre-Service and Post-Service online forms and double check with Mr. Saenz to make sure he got your email forms. Turn in Signature Time Sheet to Mr. Saenz. Check Service Learning board for verification of approved hours.
