Directions: Fill out form with all appropriate information and click on the “Submit” button at bottom. Mr. White will contact you by email or posting when your replacement gear is ready for pick-up.
Phoenix Military Academy Uniform Definitions
All items below are separated in accordance to the items funding source, to help to clarify the responsibilities of both the school’s JROTC program and the participating cadet.
Army Items - These items are provided by the U.S. Army on loan (at no cost) to the participating cadet. Student is required to return items upon request of Military Supply Officer, Army Instructor, or School Administrator. Failure to return these items will result in a cost to the student or the parent by the U.S. Department of the Army:
- 1 All weather coat with liner (trench coat)
- 1 Poly Wool Coat Green (Class A Jacket)
- 3 Army Green Shirts
- 2 Trousers/ Slacks/ Skirts
- 1 Windbreaker Black with liner
School Item - These items are provided to the participating cadet by the school and may or may not be included in the school fees. These items are required to be worn according to the cadet manual. Below are listed items indicating which items must be returned to the school upon leaving the program:
- Parka Coat with hood Wool (P Coat, West Point Coat)*
- Polo shirt (Phoenix Military Academy)*
- Black Book Bag*
- Physical Training Uniform*
- Black Army Sweater- item must be returned to school
- Navy Blue Beret- item must be returned to school
- Dress Blues Complete uniform and accessories- item must be returned to school
Non- Recoverable Item - These items are provided by the Department of the Army at no cost to the participating cadet. Cadet is responsible for the upkeep of these items. If the student misplace item, he/ she must replace them by their own means.The student is not required to return these items upon leaving the JROTC program:
- Black Shoes- Cadets Let 2-4 are permitted to exchange shoes once within a school year
- Belt*
- Buckle*
- Neck tie/ tab*
- Garrison Cap*
- Black Gloves*
- Black Socks*
- Name Plate*
* these items can not be exchanged once issued
